2. A mandatory activity fee in the amount of $100 must be submitted. Please do not remit payment until after tryouts have concluded and the official team has been made. This fee is non-refundable and will not be returned if you pay this fee prematurely and your child does not make the team or decides to leave the team. Payment of this fee should be submitted online AFTER your child makes the team at: https://schoolpaymentportal.com/Default.aspx
**If you prefer to make a payment via check, please make it payable to “Regional School District 13 General Fund”. All payments may be handed in to the main office @ CRHS only AFTER your child has made their sport team. Please refer to the Pay to Participate Letter for family max and individual max.
3. A Sports Health Assessment Record (Physical Examination) completed by the Physician and Parent/Guardian must be submitted directly to the Coginchaug nurse, Pam Branciforte. In order to be considered valid, the date of the physical examination can be no more than 13 months old. You must submit a copy of your latest health physical if it is not current.
4. Regional School District 13 has provided its Athletic program access to concussion management system called Impact. All CRHS student-athletes must have completed a baseline ImPact test within the past two years with school personnel. This must be completed prior to the first date of competition. The baseline test is re-administered every other year. This information will be used in the event a student athlete sustains a concussion throughout the school year.
5. All eligibility requirements set by the CIAC must be satisfied must be met in order to play a sport in Regional School District 13 (Please Refer to Student-Athlete handbook).
