Instructions on How to REGISTER FOR a Sport at CRHS
In order to participate in any interscholastic sport at CRHS, the following requirements must be met prior to your child beginning tryouts or practices for each individual sport:
1. Completion of Family ID Registration for Fall Sports 2024 Interscholastic Athletics
- Please use the following link if you are looking to register for Unified Sports. Please note, this link will be active for the entire school year. Family ID Unified Sports Registration link
- Parents, please remember that ALL Unified Sports participants will require an updated physical examination within the past 13 months. If you have had one updated, please remember to drop off a hard copy to the school Nurse at your earliest convenience.
- Unified Sports activities will not begin until after the first day of school. If you have any questions regarding Unified, please refer to our Unified Sports page for more information.
2. A Sports Health Assessment Record (Physical Examination) completed by the Physician and Parent/Guardian must be submitted (via Hardcopy) directly to the Coginchaug School Nurse, Pamela Branciforte.
- **For the Fall Athletic Season 2024 - Please note as some dates and processes have changed. CIAC has changed start dates of the fall sport season due to the modifications of the summer practices rules instituted in June 2024. Therefore, ALL prospective Fall student athletes should submit a medical / physical examination (hard copy) to the School Nurse NO LATER than Friday August 9th. Submitting a physical no later than this date will help ensure that your child is able to begin fall sport practice on Monday August 19th.
- **This is different than the medical guidelines for Summer Athletic Programs. It may be very likely that your child was cleared for summer athletic programs, and would not be cleared for fall sport programs based on the differences in helath documentation required for each program.
- The CIAC Medical Committee recommends dates of the physical examination to 13 months. This guideline is also supported by the Regional School district medical advisor. You must submit a hard copy of your latest health physical if it is not current. No exceptions can be made. There is a box in the CRHS foyer for you to drop off updated physicals.
3. Regional School District 13 has provided its Athletic program access to a concussion management system called Impact. All CRHS student athletes must have completed a baseline ImPact test as directed by school personnel, typically the Athletic Trainer. This must be completed prior to the first date of the competition. The baseline test is re-administered every other year in grades 9 & 11. This information will be used in the event a student athlete sustains a concussion throughout the school year.
4. All eligibility requirements set by the CIAC must be met in order to play a sport in Reg School District 13. (Please Refer to CRHS Student Athlete handbook tab).
