In order to participate in any interscholastic sport at CRHS, the following requirements must be met prior to your child beginning tryouts or practices for each individual sport:
We are very excited to announce that the Coginchaug Athletic Department has partnered with FinalForms, an online forms and data management service. FinalForms allows you to complete and sign athletic participation forms for your students. The most exciting news is that FinalForms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice!
FinalForms also pre-populates information wherever possible, for each of your students, saving
you time. You may review your data at any time to verify it is current. You will be required to sign your forms once per year and after any update.

We are asking the parents/guardians of all students planning to play sports to create accounts in FinalForms and register your student(s).
1. Completion of FinalForms Sport Registration for Interscholastic Athletics. The active registration link is the same for for each season, but you will not be able to register for other seasons unless the below dates.
- Fall Sport Registration will open up on Friday July 10, 2026
- Winter Sport Registration will open up on / about October 30, 2026
- Spring Sport Registration will open up on / about January 30, 2027
2. Unified Sports FinalForms registration link - Active entire year 2026-2027.
- Link will be posted closer to th estart of the 2026-2027 School year
- Parents, please remember that ALL Unified Sports participants will require an updated physical examination within the past 13 months. If you have had one updated, please remember to drop off a hard copy to the school Nurse at your earliest convenience.
- Unified Sports activities will not begin until after the first day of school. If you have any questions regarding Unified, please refer to our Unified Sports page for more information.
3. A Sports Health Assessment Record (Physical Examination) completed by the Physician and Parent/Guardian must be submitted (via FinalForms) directly to the FinalForms Registration page. You may also submit a Hardcopy directly to the Coginchaug School Nurse, Pamela Branciforte. Please DO NOT email an electronic copy to School Nurse / Athletic Trainer.
- **For the Fall Athletic Season 2026 - Please note as some dates and processes have changed. CIAC has changed start dates annually. Therefore, ALL prospective Fall student athletes should complete their FinalForms registration and SUBMIT a medical / physical examinations NO LATER than Friday August 7th. If you are submitting a (hard copy) to the School Nurse, please adhere to the same timeline. Submitting a physical no later than this date will help ensure that your child is able to begin on the first day of their sport practice.

- Football will begin on Monday August 17th
- All ofther sports (Cheerleading, Cross Country, Soccer, Volleyball) will begin on Monday August 24th.
- **This is different than the medical guidelines for Summer Athletic Programs. It may be very likely that your child was cleared for summer athletic programs, and would not be cleared for fall sport programs based on the differences in helath documentation required for each program.
- The CIAC Medical Committee recommends dates of the physical examination to 13 months. This guideline is also supported by the Regional School district medical advisor. You must submit a hard copy of your latest health physical if it is not current. No exceptions can be made. There is a box in the CRHS foyer for you to drop off updated physicals.
4. Regional School District 13 has provided its Athletic program access to a concussion management system called Impact. For the SY 2026-2027 we will be using a new program called SWAY. More information regarding this program will be sent out later this summer. Student athletes will be administered a baseline as directed by school personnel, typically the Athletic Trainer. This must be completed prior to the first date of the competition. The baseline test is re-administered every other year in grades 9 & 11. This information will be used in the event a student athlete sustains a concussion throughout the school year.
5. All eligibility requirements set by the CIAC must be met in order to play a sport in Reg School District 13. (Please Refer to CRHS Student Athlete handbook tab).
